Jessica Hsu

Hunter College of the City University of New York

Jessica Hsu received her Bachelor of Science in Nursing at James Madison University, where she graduated cum laude as an honors scholar in 2011. For her honors thesis, she completed "A Literature Review of Community Health Workers in the United States." Ms. Hsu currently works in a large academic health center on a 33 bed unit that serves patients with chronic illnesses requiring kidney and liver solid organ transplants. She currently is the chair of the unit's Nursing Practice Standards and Protocol Committee and has participated in interdisciplinary projects on Value Based Medicine in the care of patient's undergoing laparoscopic colon resection. Experience with a patient population that requires long term coordination of care among inpatient and outpatient services opened her eyes to the potential gaps in care in the transition from the hospital to home. She is currently pursuing a Doctor of Nursing Practice degree in the Adult and Geriatric Nurse Practitioner track. Ms. Hsu will utilize knowledge of healthcare systems, health policy and leadership skills to build practice protocols to increase primary care nurse practitioners' presence in acute care settings to seamlessly transition patients from acute to primary care. Her hope is to equip primary care providers to manage the complex health needs of patients discharged from acute care settings.
