Addressing the Healthcare Challenges of the 21st Century through an Innovative Practice Model

The program, Addressing the Healthcare Challenges of the 21st Century through an Innovative Practice Model, supports the establishment and operations of a Nurse Practitioner (NP), faculty-run clinic to provide primary care to under-served individuals and older adults. Through inter-professional collaboration between faculty from New York University Colleges of Nursing and Dentistry, dental patients are referred to the NP faculty clinic as well as drawn from the community. Using a comprehensive care model, the NP faculty practice focuses on patient-centered, disease-based clinical management, health promotion and disease prevention. Specific targeted patient outcomes include increased access to primary healthcare services; continuity of care; improved health status; risk factor reduction; patient satisfaction; and patient retention.
The NP faculty clinic also provides a teaching venue for faculty to provide health professions students a clinical experience focused on health promotion and disease prevention. In addition, the NP clinic program serves as a feasibility pilot for an innovative nursing practice model.
Monitoring and evaluation of the project include patient and staff measures, health professions student assessment data as well as standard business metrics and outcomes of the clinic operations.
